Pharmacotherapy (adjunctive, second-line)

- Factitious disorder: no drug therapy; avoid aggressive confrontation, protect the patient from further self-harm, and manage comorbid personality pathology. Factitious disorder imposed on another is child abuse and mandates a report to child protective services.
Avoid
- Opioids and benzodiazepines: no benefit, high dependence risk.
- Elective invasive procedures without objective indication.
- Repeated bare reassurance: transiently anxiolytic, long-term symptom-reinforcing.
Iatrogenic complications (the most important category)
- Procedural and surgical harm: repeated laparoscopy, endoscopy, or exploratory surgery for unexplained pain leads to adhesions, bowel injury, and anesthetic complications. Signalled by an operative history disproportionate to documented pathology.
- Cumulative radiation and contrast exposure: serial CT imaging carries malignancy risk; contrast-associated acute kidney injury presents as a creatinine rise after imaging.
- Opioid dependence and overdose — emergency: escalating doses for pain that has no nociceptive generator; miosis, somnolence, and respiratory depression signal overdose. Benzodiazepine co-prescription compounds risk.
- Polypharmacy from sequential specialist trials: drug–drug interactions, anticholinergic burden, QT prolongation. TCAs are lethal in overdose (wide QRS, arrhythmia, seizures) — relevant given high comorbid depression.
- Serotonin syndrome: with SSRI/SNRI plus tramadol or triptans; clonus, hyperreflexia, hyperthermia, agitation — emergency.
Complications of the disorder itself
- Missed organic disease from diagnostic anchoring — emergency: once labeled, new pathology is under-investigated. The signal is a change in symptom character, objective findings, or abnormal laboratory data.
- Major depression and suicide — emergency: chronic pain, disability, and adversarial care relationships drive suicidal ideation; always ask directly.
- Disability, unemployment, and social isolation: avoidance behavior prevents extinction learning and entrenches the sick role.
- Deconditioning and immobility complications in functional neurological disorder: prolonged functional paralysis causes contractures, disuse atrophy, pressure injury, and venous thromboembolism. Functional seizures mistaken for status epilepticus can lead to unnecessary intubation and ICU admission — a preventable iatrogenic emergency.
Complications specific to factitious disorder — frequently emergencies:
- Sepsis and abscess from self-injected material: polymicrobial or enteric organisms in an otherwise unexplained bacteremia.
- Surreptitious insulin use: hypoglycemia with high insulin and low C-peptide; sulfonylurea ingestion gives high insulin with high C-peptide plus a positive drug screen.
- Self-phlebotomy or anticoagulant ingestion: unexplained iron-deficiency anemia or coagulopathy correcting off the ward.
- Factitious disorder imposed on another: recurrent illness in a child that resolves during caregiver separation — reportable child abuse.
- DSM-5-TR shifted the emphasis from "medically unexplained" to excessive cognition and behavior: a patient with real coronary disease can still have somatic symptom disorder if the preoccupation is disproportionate. Duration is ≥6 months. This is the single most tested conceptual change.
