Functional Neurologic Symptom Disorder (FNSD), formerly termed conversion disorder, is a condition characterized by neurologic symptoms incompatible with recognized neurologic or medical disease. The disorder affects approximately 2-5% of neurology clinic patients and 16% of primary care patients, though prevalence varies with diagnostic stringency and population studied. FNSD is not a diagnosis of exclusion but rather a positive diagnosis based on internally inconsistent neurologic findings and characteristic clinical features. The distinction from "non-organic" or "psychogenic" disease is now recognized as outdated; modern neurobiologic evidence supports genuine alterations in neural processing and motor control. FNSD carries significant morbidity including functional disability, healthcare utilization burden, and psychological distress. Early diagnosis and multidisciplinary intervention substantially improve outcomes compared to delayed recognition.
FNSD involves disrupted integration between intentional motor planning and automatic motor execution, with objective neurobiologic correlates rather than purely psychogenic mechanisms:
- Dysfunctional motor network integration: Functional neuroimaging demonstrates hypoactivity in primary motor cortex (M1) during attempted movement of affected limbs, paradoxically with increased activity in anterior cingulate and prefrontal regions. This reflects excessive top-down inhibitory control over automatic motor pathways. The supplementary motor area (SMA) and premotor cortex show reduced connectivity, disrupting the normal hierarchical control of movement from intention to execution.
- Abnormal predictive coding and sensorimotor incongruence: The cerebellum's role in predictive modeling is compromised, preventing proper integration of motor efference copy with sensory feedback. This leads to the brain treating voluntary movements as externally generated (similar to passive movement perception), triggering heightened sensorimotor conflict and suppression of voluntary output. Basal ganglia dysfunction reduces automatic motor facilitation normally suppressing inhibitory pathways.
- Altered limbic-motor coupling and emotional salience processing: Enhanced amygdala-motor network connectivity links emotional arousal to motor suppression. Heightened interoceptive awareness (insular cortex hyperactivity) amplifies attention to bodily sensations and symptom-related threat perception. The default mode network hyperactivity during motor attempts reflects self-focused attention rather than task engagement, maintaining the cycle of motor suppression.
- Disrupted descending inhibitory control pathways: Altered dorsolateral prefrontal cortex (dlPFC) function impairs top-down regulation of threat-related motor inhibition. The anterior insula and dorsal anterior cingulate show increased resting-state connectivity, driving excessive error monitoring and behavioral inhibition. Reduced GABAergic inhibitory tone in motor circuits may lower the threshold for symptom expression.
- Neurotransmitter dysregulation: Dysfunction in dopaminergic mesolimbic pathways and serotonergic systems contributes to reduced motivation for motor recovery and heightened anxiety sensitivity. Altered endogenous opioid system function may enhance somatic symptom amplification.

- Iatrogenic factors (excessive investigation, diagnostic labeling, sick role reinforcement)
Demographic and social factors
- Female predominance (2-5:1 ratio), though increasing male recognition
- Age 20-50 years most common
- Lower socioeconomic status and reduced educational attainment (variable)
- Cultural factors influencing symptom expression (somatic symptoms more prominent in some cultures)
- Positive family history of FNSD or psychiatric disease
- Comorbid health anxiety and symptom hypervigilance
Neurobiologic vulnerability factors
- Genetic predisposition to emotional dysregulation
- Enhanced autonomic nervous system reactivity
- Early-life stress effects on limbic system development
- Reduced resilience and maladaptive coping strategies
Motor manifestations (most common, ~80% of cases)
- Weakness or paralysis: Typically non-anatomical distribution (e.g., entire leg vs. L5 dermatome); often with inconsistent severity and selective involvement of antigravity muscles while preserving gravity-assisted movements
- Tremors: Irregular, variable frequency (often 4-7 Hz); inconsistent amplitude and direction; diminished with distraction; often entrains to passive limb movement frequency
- Dystonia: Exaggerated, bizarre postures; inconsistent and changeable; unlike typical organic dystonia, often improves with emotional reassurance
- Gait disturbance: Highly variable patterns including "astasia-abasia" (inability to stand/walk despite intact strength); "stiff-legged" walking; sudden buckling of knees; improves during distraction or backward walking
- Speech/voice disorders: Dysphonia (hoarse, breathy voice), dysarthria, stuttering; inconsistent voice quality; normal palatal elevation with phonation
Sensory manifestations (50-60%)
- Sensory loss in non-anatomical distributions: stocking-glove (contradicting dermatome/peripheral nerve pattern); hemibody with sharp midline demarcation; sharply demarcated patches
- Paresthesias often in affected motor regions
- Normal vibration and proprioception despite reported sensory loss (dissociation pattern)
Special sensory dysfunction
- Blindness or visual field defects: Non-physiologic patterns (tunnel vision with normal peripheral vision on confrontation; vertical splitting instead of hemianopia)
- Deafness or auditory dysfunction: Inconsistent responses to sound; preserved acoustic startle
- Olfactory/gustatory disturbances: Often with secondary complaint patterns
Seizures (15-20% of FNSD patients)
- Distinguishing features from epileptic seizures: Maintained consciousness with apparent responsiveness (sometimes); asynchronous thrashing movements; side-to-side head movement; pelvic thrusting; crying/vocalizations during event; lack of tongue biting or incontinence; normal post-event cognition
- Provoked by emotional stress or anxiety
- Normal EEG during event (critical distinguishing feature)
Associated features
- Inconsistent or changing clinical presentation between visits
- Collapsing resistance to passive movement (give-way weakness)
- Magnon sign: Sudden collapse of support when released during strength testing
- Hoover's sign: Contralateral hip extension weakness that normalizes during contralateral leg flexion against resistance (patient must use hip extensors ipsilaterally)
- Regional distribution: Multiple systems often affected; diffuse, non-anatomical patterns
- Symptoms triggered or exacerbated by attention/observation; improvement with distraction
- Marked functional disability disproportionate to objective neurologic findings
Cognitive presentation
- Normal cognition with intact insight (some patients deny primary psychological causation despite evidence)
- Health anxiety and symptom preoccupation
- Depressive or anxiety symptoms concurrent
Clinical assessment (positive diagnosis, not exclusionary)
Diagnosis requires: (1) One or more neurologic symptoms affecting voluntary motor or sensory function; (2) Clinical findings inconsistent with recognized neurologic disease; (3) Not explained by another medical or mental disorder; (4) Functional impairment or distress; (5) Symptom persistence ≥4 weeks (DSM-5 criteria).
Neurologic examination findings supporting diagnosis
- Hoover's sign: Bilateral hip weakness examination where contralateral hip extension normalizes when patient flexes the contralateral hip against resistance; indicates intact neural pathways with inconsistent output
- Collapsing or "give-way" weakness: Abrupt loss of resistance during strength testing rather than consistent weakness throughout range; suggests inconsistent voluntary effort
- Non-anatomical sensory level: Stocking-glove distribution or sharp midline demarcation inconsistent with dermatome/peripheral nerve anatomy
- Magnon sign: Sudden collapse when passive support released during strength testing
- Splitting of vision fields: Tunnel vision on confrontation where peripheral vision appears preserved
- Entrainment phenomenon: Tremor frequency matching external passive movement rate
- Inconsistent findings: Symptom severity and distribution varying between examinations
- Improvement with placebo or distraction: Functional improvement during cognitive engagement unrelated to symptom
- Regional (multi-system) involvement: Multiple seemingly unrelated neurologic domains affected
Diagnostic investigations (to exclude organic disease)
Laboratory studies
- CBC, comprehensive metabolic panel, thyroid function (TSH): Rule out metabolic/endocrine etiologies
- Vitamin B12, folate, rapid plasma reagin (RPR): Screen for nutritional/infectious causes
- Anti-neuronal antibodies (anti-NMDA, anti-LGI1, anti-GABA-B): If seizures present, rule out autoimmune encephalitis
- Erythrocyte sedimentation rate (ESR), C-reactive protein: Screen for inflammatory disease
- Additional testing only if atypical features or specific clinical concern (e.g., lumbar puncture if CNS infection suspected)
Electrodiagnostic studies
- Electromyography (EMG)/Nerve conduction studies (NCS): Normal in sensory FNSD; helpful to exclude myopathy, neuropathy, neuromuscular junction disease
- EEG: Normal background with normal reactivity; critical finding during reported seizures (absence of ictal activity distinguishes from epilepsy)
- Absence of abnormalities typical of organic disease
Neuroimaging
- MRI brain with and without contrast: Normal structural imaging; no lesions, demyelination, or atrophy accounting for symptoms; necessary to exclude stroke, tumor, demyelinating disease, Charcot-Marie-Tooth if neuropathy suspected
- Functional neuroimaging (research/specialized centers): fMRI shows reduced primary motor cortex activity with preserved premotor/prefrontal activation during attempted movement; increased default mode network activity
- MRA cervical/intracranial: If vascular event suspected; normal if FNSD
- Spine imaging: If myelopathy features present; normal in FNSD
Special diagnostic maneuvers and tests
- Babinski maneuver with distraction: Maintained normal plantar flexion during distraction when patient has reported extensor plantar response
- Sensory testing for midline splitting: Sharp demarcation at body midline during sensory examination (dermatomally inconsistent)
- Monozygotic twin study sign: Reported symptoms in one area suddenly affecting contralateral identical location
- Auditory startle/acoustic blink reflex: Preserved in functional hearing loss (rules out organic deafness)
- Doll's eye maneuver: Normal oculovestibular reflex in functional blindness
- Optokinetic nystagmus: Preserved in functional visual loss
Exclusion of organic disease
Investigation should exclude conditions that mimic FNSD including: Multiple sclerosis, Guillain-Barré syndrome, myasthenia gravis, myositis, Parkinson disease, dystonia, epilepsy, stroke, spinal cord compression, autoimmune encephalitis, toxic/metabolic encephalopathy, systemic lupus erythematosus (SLE), sarcoidosis, vasculitis, paraneoplastic syndromes.
DSM-5 Diagnostic Criteria for Functional Neurologic Symptom Disorder
(A) One or more symptoms of altered voluntary motor or sensory function
(B) Clinical findings incompatible with recognized neurologic or medical disease
(C) Not better explained by another medical or mental disorder
(D) Causes clinically significant distress or functional impairment
(E) Duration ≥4 weeks (or meets criteria for acute presentation if 1-4 weeks with severe symptoms/rapid onset)

Foundational approach: Multidisciplinary, early intervention with positive framing
Treatment efficacy increases substantially with early diagnosis (within months vs. years). Key principle: Reframe FNSD as treatable neurobiologic condition (not malingering or purely psychological) while addressing underlying contributing factors.
First-line: Behavioral and psychological therapies
- Physical rehabilitation/Physiotherapy: Gold-standard evidence-based approach; graduated motor retraining emphasizing task-specific movements, postural control, and gait retraining. Therapists skilled in FNSD recognition essential. Goals focus on functional improvement rather than "proving" organic pathology. Sessions typically 2-3 weekly for 8-12 weeks. Efficacy: 50-70% substantial improvement with intensive programs.
- Cognitive-behavioral therapy (CBT): Targets maladaptive illness beliefs, excessive symptom focus, avoidance behaviors, and emotional dysregulation. Therapists integrate understanding of symptom mechanisms (predictive coding failure, motor suppression). Evidence supports 10-20 sessions for symptom improvement. Efficacy: 40-60% meaningful response.
- Psychodynamic psychotherapy/Insight-oriented therapy: Addresses unconscious psychological conflicts contributing to symptom expression, particularly effective when clear temporal relationship to trauma/stressor exists. 12-30 sessions typically required. Efficacy: 30-50% responders.
- Acceptance and Commitment Therapy (ACT): Newer evidence-based approach emphasizing acceptance of symptoms while pursuing valued activities. Reduces symptom focus and improves functional engagement. Efficacy: 50-65% improvement in function.
- Mindfulness-based stress reduction (MBSR): Group-based 8-week program reducing emotional reactivity and symptom amplification through attention regulation. Efficacy: 40-50% of participants show sustained improvement.
Patient education and reassurance (essential first step)
- Clear explanation of FNSD diagnosis utilizing brain-imaging findings and demonstration of inconsistent neurologic findings
- Reassurance regarding prognosis: 50-85% achieve significant functional improvement with treatment; symptoms are real and reversible
- Explanation of predictive coding/motor control mechanisms in accessible language
- Avoid reinforcement of sick role; encourage active participation in rehabilitation
- Discussion of normal neuroimaging and absence of progressive degenerative disease
- Address health anxiety: Establish that continued investigation without diagnosis worsens outcomes
Pharmacologic treatment: Adjunctive role (not first-line monotherapy)
- Selective serotonin reuptake inhibitors (SSRIs): Primary indication for comorbid depression, anxiety, or panic disorder (present in 50-80% of patients). Agents: sertraline 50-200 mg daily, escitalopram 10-20 mg daily, paroxetine 20-40 mg daily. Efficacy in FNSD: Modest when used alone; improves symptoms in 30-40% related to mood/anxiety improvement. Mechanism: Enhances dlPFC inhibitory tone, reduces amygdala reactivity.
- Serotonin-norepinephrine reuptake inhibitors (SNRIs): Venlafaxine (75-225 mg daily) or duloxetine (30-60 mg daily) particularly for patients with chronic pain comorbidity. Evidence: Similar to SSRIs; 30-40% response.
- Benzodiazepines: Avoided as monotherapy (addiction risk, reinforcement of sick role); limited role for acute anxiety exacerbations. Short courses (2-4 weeks) may facilitate entry into rehabilitation. Agents: lorazepam 0.5-2 mg daily divided dosing only.
- Tricyclic antidepressants: Amitriptyline 10-50 mg nightly for pain/anxiety; anticholinergic effects limit use. Evidence: Modest (20-30% response); less favorable side-effect profile.
- Antipsychotics: Quetiapine or aripiprazole in severe cases with prominent anxiety or emotional dysregulation; limited evidence. Reserved for treatment-resistant cases.
- Botulinum toxin: Limited role in functional dystonia if disabling; some case series suggest benefit (10-20% sustained improvement) in severe cases when combined with rehabilitation.
Intervention timing and structure
- Acute presentation (1-4 weeks): Intensive multidisciplinary program combining daily physiotherapy, supportive psychotherapy, and psychoeducation. Early recognition predicts best outcomes.
- Subacute (1-6 months): Weekly PT combined with weekly CBT or psychodynamic therapy; SSRI initiation if comorbid psychiatric illness.
- Chronic (>6 months): Requires more intensive/longer duration intervention; may need inpatient rehabilitation program; address chronicity-related deconditioning and catastrophic thinking.

Disease-related complications
- Secondary disuse changes: Prolonged non-use of a functionally weak limb produces deconditioning, muscle atrophy, joint contracture, and osteopenia. The signal is a limb that has become fixed rather than variable — loss of the inconsistency that defined the original examination — and the appearance of a true, structural deficit layered on the functional one.
- Complex regional pain syndrome and fixed dystonia: Immobilized, splinted, or repeatedly injured limbs may develop allodynia, edema, and trophic/color change. Suspect it when pain and autonomic skin change outstrip the motor complaint.
- Falls and injury: Astasia-abasia and sudden knee buckling cause head strike and fracture; each injury reinforces avoidance and further immobility.
- Immobility cascade: Wheelchair or bed dependence risks venous thromboembolism and pressure injury — unilateral limb swelling or hypoxia in a chronically immobile FNSD patient is an emergency and must be worked up conventionally, not attributed to the functional diagnosis.
- Psychiatric decompensation: Depression, anxiety, and suicidality accompany the high comorbidity burden described earlier; suicidal ideation is an emergency requiring immediate risk assessment. The APA and USPSTF both support systematic depression screening in this population.
- Missed or evolving structural disease: FNSD coexists with neurologic disease, so a new fixed deficit, first-ever fever with focal signs, or an abrupt vascular-pattern syndrome demands standard stroke/infection evaluation. Modern series place the rate of later diagnostic revision at only a few percent, but the individual patient can still have a second illness.
Treatment-related complications
- Iatrogenic harm from over-investigation: Repeat imaging, angiography, lumbar puncture, and surgery expose patients to procedural risk and entrench illness beliefs.
- Pseudostatus epilepticus mismanagement: Prolonged functional seizures misread as refractory status lead to escalating benzodiazepines, intubation, and ICU admission — an emergency of iatrogenesis. Clues are preserved oxygenation, normal lactate/acidosis profile, and no ictal EEG correlate; ILAE/American Epilepsy Society guidance favors early video-EEG.
- Pharmacologic adverse effects: SSRI/SNRI hyponatremia and sexual dysfunction, TCA anticholinergic and QT effects, benzodiazepine dependence and sick-role reinforcement, and botulinum toxin–induced true weakness or dysphagia.
- Positive diagnosis, not exclusion: Per APA DSM-5-TR, criterion B requires findings incompatible with recognized disease — the exam sign makes the diagnosis. A stem that says "MRI and EEG were normal" alone is not enough; look for the internal inconsistency.
- Hoover sign is the single most tested maneuver: Voluntary hip extension is weak, yet extension returns automatically when the patient flexes the opposite hip against resistance. Mechanism: the automatic crossed-extension synergy bypasses the impaired voluntary motor pathway, proving the corticospinal tract is intact.
- Tremor entrainment: Ask the patient to tap a rhythm with the unaffected hand; a functional tremor entrains to, or stops at, the imposed frequency. Organic tremor does not.
- Best next step for suspected functional seizures: Video-EEG monitoring to capture a typical event, the diagnostic gold standard per ILAE/American Epilepsy Society. Favoring features are eyes closed with active resistance to opening, prolonged duration, asynchronous or side-to-side thrashing, pelvic thrusting, ictal crying, and preserved awareness.
- Best next step once diagnosed: Explicitly explain the diagnosis by name, show the patient the Hoover sign, and refer to FNSD-experienced physiotherapy — not another MRI. Communication of the diagnosis is itself therapeutic; repeat imaging worsens outcomes.
- The association examiners test: High rates of comorbid depression, anxiety, PTSD, and childhood adversity — but a psychological stressor is not required by DSM-5-TR, and its absence does not exclude FNSD.
- The intentionality distinction: FNSD symptoms are not feigned. Factitious disorder = deliberate deception for the sick role; malingering = deliberate deception for external gain (money, drugs, avoiding duty).
Common distractors to avoid
- La belle indifférence is neither sensitive nor specific and should not drive the answer.
- A normal or non-elevated prolactin does not exclude an epileptic seizure and is at best supportive, never diagnostic.
- Coexisting epilepsy or other neurologic disease is common and does not rule out FNSD.
