Turner syndrome is a chromosomal disorder characterized by complete or partial monosomy of the X chromosome, occurring in approximately 1 in 2,000–2,500 live female births. It represents the most common sex chromosome abnormality in females and accounts for significant morbidity related to short stature, gonadal dysgenesis, and cardiovascular complications. The condition arises from nondisjunction during meiosis or mitotic errors, resulting in 45,X karyotype in classic presentations or mosaicism (45,X/46,XX) in approximately 50% of cases. Clinical significance lies in early recognition enabling timely intervention with growth hormone and estrogen replacement therapy to optimize adult height and sexual development. Turner syndrome is frequently tested on USMLE Step 2 CK, particularly regarding diagnosis of short stature, cardiovascular screening, and management of primary amenorrhea.
Turner syndrome results from genetic and molecular consequences of complete or partial X chromosome loss, with cascading effects on growth regulation, gonadal development, and cardiac embryogenesis.
- X chromosome haploinsufficiency and growth hormone deficiency: The Xp11.2 region contains the SHOX gene (short stature homeobox-containing gene), which escapes X-inactivation and is critical for skeletal growth and bone development. Haploinsufficiency of SHOX results in the characteristic short stature seen in Turner syndrome, with loss of one functional copy reducing height by approximately 7-10 cm compared to genetically predicted height. Additionally, approximately 50% of patients with Turner syndrome have diminished growth hormone (GH) secretion, further contributing to growth failure through reduced insulin-like growth factor-1 (IGF-1) production. The mechanism involves hypothalamic-pituitary dysfunction with impaired GH-releasing hormone signaling and somatotroph dysfunction, compounding the skeletal growth deficit.
- Gonadal dysgenesis and sex hormone deficiency: The Xq13-q26 region contains genes critical for oogenesis, including the DAX1 gene and other dosage-sensitive genes involved in sex determination and meiotic progression. Complete X chromosome loss or substantial deletions of Xq regions result in failure of primordial germ cells to migrate properly and proliferate during fetal development. In classic 45,X Turner syndrome, the streak gonads consist of fibrotic connective tissue with absent or severely diminished oocytes, resulting in complete primary ovarian failure. This gonadal dysgenesis manifests clinically as elevated follicle-stimulating hormone (FSH) and luteinizing hormone (LH) due to loss of negative feedback inhibition, with FSH levels often >40 mIU/mL by adolescence. The resulting estrogen and progesterone deficiency accounts for absent or delayed menarche, primary amenorrhea, infertility, and increased cardiovascular and bone disease risk.
- Embryologic abnormalities and cardiovascular malformations: X chromosome monosomy disrupts normal gene dosage balance during early embryogenesis, particularly affecting neural crest cell migration and cardiac tissue development. The most common cardiac malformations include bicuspid aortic valve (occurring in up to 35% of patients), coarctation of the aorta (10-15% of cases, particularly associated with 45,X karyotype), hypoplastic left heart syndrome, aortic stenosis, and mitral valve prolapse. The proposed mechanism involves aberrant hemodynamic signaling during embryonic cardiogenesis and altered expression of genes involved in cardiac morphogenesis on the X chromosome. Renal and urinary tract abnormalities occur in 30-40% of patients, including horseshoe kidney, renal hypoplasia, and duplicated collecting systems, likely reflecting similar embryologic disruption during nephrogenesis.
- Skeletal dysplasia and bone metabolism alterations: Beyond SHOX haploinsufficiency, Turner syndrome results in characteristic skeletal features including short fourth and fifth metacarpals, cubital valgus (increased carrying angle), short neck with webbing due to fetal lymphedema, and micrognathia. Estrogen deficiency in adolescence and adulthood contributes to decreased bone mineral density, with accelerated bone loss if hormone replacement is delayed or inadequate. The combination of reduced peak bone mass acquisition during adolescence and increased bone turnover predisposes to osteoporosis and increased fracture risk in adulthood.
Turner syndrome results exclusively from chromosomal abnormalities; no modifiable risk factors or reversible causes exist. The etiology reflects germline errors occurring during gametogenesis or early embryonic development.
- Complete monosomy X (45,X karyotype): Represents approximately 50-60% of Turner syndrome cases and results from complete loss of one X chromosome due to nondisjunction during maternal or paternal meiosis I or II, or from early mitotic loss during embryogenesis. This accounts for the most severe phenotypic expression, including short stature, gonadal dysgenesis with complete ovarian failure, and highest risk of cardiovascular anomalies, particularly coarctation of the aorta. Classic presentation with complete monosomy occurs in younger patients and predicts more significant growth deficiency and earlier need for hormone replacement.
- Mosaic karyotypes (45,X/46,XX and variants): Approximately 40-50% of Turner syndrome cases demonstrate mosaicism with a population of cells containing both 45,X and 46,XX (or other normal sex chromosome complement), arising from post-zygotic mitotic nondisjunction. Mosaic forms typically present with milder phenotypes than pure 45,X, including greater final adult height (though still reduced), possible spontaneous puberty with residual ovarian function, and lower incidence of structural cardiac anomalies. Variant mosaicisms include 45,X/46,XY (which carries theoretical malignancy risk from Y chromosome material necessitating gonadectomy) and 45,X/46,XX/47,XXX.
- Structural X chromosome abnormalities: Deletions of portions of Xp (short arm) or Xq (long arm) account for the remaining cases, with phenotypic severity correlating with size and location of deletion. Xp deletions particularly involving SHOX region result in severe short stature; Xq deletions involving the critical Xq13-q26 region cause gonadal dysgenesis and primary ovarian failure. Isochromosome Xq (i(Xq)) represents a specific structural rearrangement where the long arm is duplicated and short arm lost, resulting in loss of SHOX and subsequent severe growth deficiency.
Turner syndrome presents with highly variable phenotypic expression ranging from subtle findings in mosaic forms to classic multi-system involvement in complete monosomy. Clinical manifestations reflect the underlying pathophysiology of growth hormone deficiency, gonadal dysgenesis, and embryologic malformations.
- Short stature (cardinal feature): Present in nearly all Turner syndrome patients, typically becoming apparent by age 3-5 years with progressive height divergence from normal growth curves. Adult final height averages 142-147 cm (approximately 4'8"-4'10") without treatment versus 163 cm in unaffected females, representing approximately 7-10 cm deficit from SHOX haploinsufficiency and approximately 5-8 cm from GH deficiency. Growth deceleration occurs with relative preservation of growth velocity in early childhood but progressive fall-off in mid-childhood, particularly evident on longitudinal growth chart tracking.
- Sexual infantilism and primary amenorrhea: Results from complete or near-complete gonadal dysgenesis with absent spontaneous pubertal development in approximately 90% of 45,X patients. Patients typically fail to develop secondary sexual characteristics (breast development, pubic/axillary hair, menarche) by expected ages, with breast development typically absent unless receiving estrogen replacement. Primary amenorrhea becomes clinically apparent by mid-teens if not recognized earlier. In mosaic forms, approximately 10-30% achieve some spontaneous puberty with irregular menses and possible (though usually subfertile) ovarian function.
- Classic facial and skeletal features: Characteristic craniofacial findings include micrognathia (small jaw), high-arched palate, hypertelorism (widely spaced eyes), low-set ears, and skeletal abnormalities including cubital valgus (increased carrying angle >15 degrees), short neck often with webbing (pterygium colli) or redundant skin folds from fetal lymphedema. Skeletal dysplasia manifests as short fourth and fifth metacarpals and metatarsals (demonstrable on hand radiographs), micrognathia, and occasionally scoliosis (occurring in 10-15% of patients).
- Lymphatic abnormalities and edema: Fetal and neonatal presentation may include cystic hygroma (lymphangioma) in the nuchal region (25-30% of prenatally diagnosed cases), resulting in characteristic webbed neck appearance when resolved, generalized fetal edema (hydrops fetalis in severe cases), and left-sided cardiac abnormalities. Peripheral lymphedema, particularly affecting hands and feet, occurs in approximately 50% of patients, potentially worsening in adulthood and predisposing to recurrent cellulitis.
- Cardiovascular abnormalities (detected on screening): Most common structural cardiac defects include bicuspid aortic valve (35% of patients, frequently clinically silent but requiring endocarditis prophylaxis recommendations), coarctation of the aorta (10-15%, particularly in 45,X patients with higher incidence around 16%), aortic stenosis, hypoplastic left heart, mitral valve prolapse (20-30% of patients), and left ventricular hypertrophy. Clinical presentation ranges from asymptomatic murmur detected on routine examination to symptomatic hypertension, reduced exercise tolerance, or hemodynamically significant cardiac lesions. Hypertension occurs in 25-30% of Turner syndrome patients even without coarctation, likely reflecting renal abnormalities and endothelial dysfunction.
- Renal and urinary tract abnormalities: Structural abnormalities identified on imaging include horseshoe kidney (occurring in 30% of patients), renal hypoplasia/agenesis, duplicated collecting systems, and hydronephrosis. Most are asymptomatic and discovered incidentally on screening ultrasound; however, may predispose to pyelonephritis, hematuria, and progressive renal insufficiency if not monitored.
- Otologic and audiologic findings: Conductive hearing loss due to chronic otitis media with effusion occurs in approximately 50-60% of children with Turner syndrome, reflecting Eustachian tube dysfunction from skeletal abnormalities and immune factors. High-frequency sensorineural hearing loss can also occur, necessitating audiologic screening and intervention to prevent speech and language developmental delays.
- Metabolic and endocrine abnormalities: Impaired glucose tolerance and type 2 diabetes mellitus occur with increased prevalence (approximately 10% by adulthood), potentially exacerbated by estrogen deficiency, obesity, and possible underlying insulin resistance. Thyroid autoimmunity manifests as elevated anti-thyroid peroxidase (anti-TPO) antibodies in approximately 15-30% of patients, with clinical hypothyroidism developing in 15-20%, requiring periodic screening and thyroid function monitoring.
- Cognitive and psychosocial manifestations: While general intelligence is typically normal in Turner syndrome, specific deficits in visuospatial processing, mathematics, social cognition, and executive function occur in a substantial minority (20-30% of patients). Patients frequently experience social difficulties, anxiety, and depression related to short stature, absent sexual maturation during adolescence, and infertility concerns, requiring psychological support and appropriate peer counseling.
Diagnosis of Turner syndrome requires integration of clinical suspicion, cytogenetic confirmation, and consideration of alternative diagnoses in the evaluation of short stature and primary amenorrhea.
- Karyotype analysis (gold standard diagnostic test): 45,X karyotype from peripheral lymphocytes definitively establishes Turner syndrome diagnosis; however, false-negative rate of approximately 10-15% occurs if mosaicism present in unsampled cell lines. Standard karyotype should be supplemented with fluorescence in situ hybridization (FISH) or chromosomal microarray if clinical suspicion remains high despite normal karyotype, as these techniques have superior sensitivity for detecting low-level mosaicism (<10% of cells). Testing should ideally include a minimum of 50 cells analyzed to optimize detection of mosaic cell lines.
- Clinical diagnostic criteria and initial evaluation: The Turner Syndrome Study Group has established diagnostic criteria requiring presence of complete or partial X chromosome monosomy combined with at least one clinical feature from growth abnormality, gonadal dysgenesis (elevated FSH >40 mIU/mL on two occasions 4 weeks apart in prepubertal girls, or primary ovarian failure), or cardiac/renal anomalies. Initial diagnostic workup should include: (1) height measurement plotted on growth curves with assessment of parental heights and genetic prediction; (2) FSH and LH levels (markedly elevated FSH >20 mIU/mL in prepubertal girls, >40 mIU/mL in adolescents indicates gonadal failure); (3) thyroid function tests (TSH, free T4) and anti-TPO antibodies given autoimmune thyroid predisposition.
- Cardiovascular evaluation mandatory at diagnosis: All Turner syndrome patients require comprehensive cardiac assessment including transthoracic echocardiography to evaluate for structural cardiac abnormalities (bicuspid aortic valve, coarctation, aortic root dimensions, valve function, ventricular function), as these findings guide long-term management and imaging surveillance. Electrocardiography (ECG) should be obtained to assess for left ventricular hypertrophy patterns or ischemic changes. Renal ultrasound is recommended as part of initial evaluation given 30-40% prevalence of structural abnormalities and implications for long-term renal function and hypertension management.
- Hearing assessment and audiologic testing: Audiometry should be performed in childhood given high prevalence of conductive hearing loss from otitis media with effusion (50-60% of patients). Otoscopic examination for fluid levels and tympanometry should precede audiometric testing. Sensorineural hearing loss screening is appropriate given risk of high-frequency hearing loss even in younger patients.
- Bone density assessment: Dual-energy X-ray absorptiometry (DEXA) of lumbar spine and proximal femur should be obtained in late adolescence or early adulthood to establish baseline bone mineral density (BMD) Z-scores. Patients frequently demonstrate BMD Z-scores 1-2 standard deviations below age-matched controls, with osteoporosis (T-score ≤-2.5) reported in 20-30% of adults not receiving adequate hormone replacement.
- Laboratory markers of gonadal dysfunction: Basal FSH levels >10-15 mIU/mL are suggestive; levels >40 mIU/mL on two occasions 4 weeks apart are diagnostic of primary ovarian failure in prepubertal/early pubertal girls. LH is typically also elevated, though less sensitive. Estradiol levels are suppressed (<20 pg/mL) reflecting absent ovarian function. Anti-Müllerian hormone (AMH) levels are typically low or undetectable, reflecting depleted ovarian reserve.
- Differential diagnosis considerations: Short stature differential diagnosis must exclude other common etiologies including growth hormone deficiency (though Turner syndrome patients can have concurrent GH deficiency), hypothyroidism (thyroid function must be normal), chronic systemic disease, nutritional insufficiency, and familial short stature (addressed by comparing to parental heights and genetic height prediction). Primary amenorrhea differential diagnosis includes Müllerian agenesis (with normal secondary sexual development and normal karyotype, distinguishing from Turner syndrome), constitutional delay of puberty (family history of late puberty, normal growth trajectory, normal FSH levels), and other causes of gonadal dysgenesis (46,XX pure gonadal dysgenesis, androgen insensitivity).
Management of Turner syndrome requires comprehensive, multidisciplinary approach addressing short stature, gonadal dysgenesis, cardiovascular abnormalities, and psychosocial needs. Treatment should be individualized based on karyotype, clinical features, and patient/family preferences.
- Growth hormone therapy (first-line for short stature): Recombinant human growth hormone (rhGH) is the primary therapeutic intervention to optimize adult height, approved for Turner syndrome at doses of approximately 0.05-0.1 mg/kg/day administered by subcutaneous injection (typically 6-7 times weekly or as daily injections). Response is optimal when initiated early (ideally by age 4-6 years) and continued through adolescence until epiphyseal fusion. Mean height gain with GH monotherapy ranges from 4-7 cm over final adult height. GH should be continued throughout the growth phase, with transition to alternative dosing schedules or discontinuation only after epiphyseal fusion documented by bone age assessment or achievement of near-final adult height. Monitoring during GH therapy requires regular (every 3-6 months) assessment of height velocity, growth trajectory, and adjustment
Cardiovascular — the leading cause of excess mortality
- Aortic dissection/rupture (emergency): intrinsic medial abnormality of the aorta compounded by bicuspid aortic valve, coarctation, and hypertension. Because patients are short, absolute aortic diameter underestimates risk — the international Turner syndrome consensus guidelines (2016 Cincinnati meeting) and ACC/AHA aortic disease guidance advise indexing aortic diameter to body surface area (aortic size index) and following it with serial echocardiography or MRA. Acute tearing chest or interscapular pain, pulse/blood-pressure differential, or new aortic regurgitation demands immediate CT angiography and surgical consultation.
- Pregnancy-associated dissection (emergency): gestational volume expansion plus a diseased media. Pregnancy — usually via donor oocytes — is high risk. Per the 2016 international consensus, pregnancy is contraindicated with prior aortic dissection or an aortic size index above roughly 2.5 cm/m²; bicuspid aortic valve, repaired coarctation, and hypertension are additional risk factors that lower the acceptable threshold and mandate pre-conception imaging with high-risk maternal–fetal medicine/cardiology co-management.
- Hypertension (often without coarctation) and re-coarctation after repair: signaled by arm–leg gradient or brachiofemoral delay.
Endocrine, autoimmune, and neoplastic
- Gonadoblastoma/dysgerminoma: occult Y-chromosome material in mosaic karyotypes drives malignant transformation of streak gonads; detection of Y material prompts prophylactic gonadectomy.
- Hashimoto hypothyroidism and celiac disease: an incompletely understood predisposition to autoimmunity, possibly related to X-chromosome gene dosage; the practical point is periodic TSH and tissue transglutaminase IgA (with total IgA) screening, prompted also by growth deceleration despite GH.
- Osteoporosis and fragility fracture: failure of estrogen-dependent peak bone mass accrual.
- Impaired glucose tolerance, type 2 diabetes, dyslipidemia, hepatic steatosis with transaminase elevation.
Otologic
- Recurrent otitis media, effusion, and cholesteatoma from eustachian tube dysfunction, causing progressive conductive hearing loss; sensorineural loss may be superimposed.
Treatment-related
- Growth hormone: idiopathic intracranial hypertension (headache, papilledema — urgent fundoscopy; lumbar puncture with opening pressure if papilledema is present), slipped capital femoral epiphysis (limp, hip or referred knee pain), progression of scoliosis, and worsened insulin resistance.
- Estrogen replacement: premature epiphyseal fusion compromising final height if started too early or at adult doses; venous thromboembolism; endometrial hyperplasia if estrogen is given unopposed — progestin must be added once breakthrough bleeding occurs or after roughly 2 years of estrogen.
- Short stature + primary amenorrhea + elevated FSH/LH: the classic stem. Gonadal dysgenesis is a leading cause of primary amenorrhea; the hypergonadotropic pattern separates it from constitutional delay and hypothalamic amenorrhea, in which gonadotropins are low or normal.
- Single best next step: karyotype in any girl with unexplained short stature, delayed puberty, or primary amenorrhea. Once Turner syndrome is confirmed, the next steps are echocardiography and renal ultrasound, per the 2016 international Turner syndrome guidelines — these are mandatory at diagnosis, not optional.
- Newborn buzzwords: lymphedema of the dorsa of the hands and feet, nuchal cystic hygroma on prenatal ultrasound, webbed neck (pterygium colli), and widely spaced nipples with shield chest. A neonate with unexplained pedal edema plus a murmur is Turner syndrome until karyotyped.
- The association examiners test most: bicuspid aortic valve (most common cardiac lesion) and coarctation — look for brachiofemoral delay, upper-extremity hypertension, or rib notching — because they drive lifelong aortic dilation and dissection risk.
- Noonan syndrome is the classic distractor: normal karyotype (RAS/MAPK pathway mutations), affects both sexes, and features right-sided lesions (dysplastic pulmonic stenosis, hypertrophic cardiomyopathy) rather than Turner's left-sided obstructive lesions.
- Do not pick intellectual disability: general IQ is typically normal; the deficit is visuospatial/nonverbal.
- Y-chromosome material on karyotype/FISH is the trigger for prophylactic gonadectomy (gonadoblastoma risk) — a favorite twist on a "mosaic" stem.
- Estrogen restores secondary sexual characteristics and protects bone; it does not restore fertility. Pregnancy, if pursued, is by donor oocyte and requires pre-conception aortic imaging given dissection risk.
- Karyotype trivia: 45,X has no Barr body, and most 45,X conceptions end in first-trimester loss — Turner syndrome is the most common chromosomal abnormality in spontaneous abortion despite being uncommon at birth.
