Sjögren's syndrome is a chronic autoimmune disorder characterized by lymphocytic infiltration and destruction of the lacrimal and salivary glands, resulting in progressive xerophthalmia (dry eyes) and xerostomia (dry mouth). It is the second most common autoimmune rheumatologic disease after rheumatoid arthritis, with a prevalence of approximately 0.5-1% in developed countries and a striking female predominance (9:1 ratio). The disease presents in two forms: primary Sjögren's syndrome (pSS), occurring in isolation, and secondary Sjögren's syndrome (sSS), developing in the context of another autoimmune disease such as rheumatoid arthritis, systemic lupus erythematosus, or systemic sclerosis. Sjögren's syndrome is clinically significant because of its substantial morbidity from sicca symptoms, the risk of systemic manifestations affecting multiple organs, and the increased risk of lymphoma, making early recognition and management critical for medical practice and board examinations.
The pathogenesis of Sjögren's syndrome involves a complex interplay of genetic predisposition, environmental triggers, and adaptive immune dysregulation that culminates in tissue-specific autoimmunity directed against the exocrine glands.
- Genetic susceptibility and HLA association: Sjögren's syndrome demonstrates strong association with HLA-B8, HLA-DR3, and HLA-DQ2 alleles, particularly in European populations. These MHC molecules present epitopes derived from the autoantigens Ro52 and La (SS-B) to autoreactive T cells, initiating the adaptive immune response. The genetic basis is polygenic, with genome-wide association studies identifying polymorphisms in genes encoding immunoregulatory proteins (STAT4, IRF5, BLK) and molecules involved in B cell activation and survival. This explains why Sjögren's syndrome shows familial clustering and ethnic variation in disease prevalence and antibody patterns.
- Loss of immune tolerance and B cell hyperactivation: The cardinal pathologic feature is breakdown of central and peripheral tolerance mechanisms, leading to accumulation of autoreactive B and T cells in affected tissues. In the normal state, regulatory T cells (Tregs) expressing FOXP3 and producing IL-10 suppress autoreactive lymphocytes; in Sjögren's syndrome, Tregs are numerically reduced and functionally impaired. This permits escape of high-affinity B cells specific for Ro52 and La/SS-B antigens, which produce pathogenic IgG autoantibodies. B cells become hyperactivated through aberrant expression of BAFF (B-cell activating factor of the TNF family), produced by salivary and lacrimal epithelial cells themselves, creating a positive feedback loop. Notably, circulating B cells in Sjögren's syndrome demonstrate signs of chronic antigenic stimulation, including somatic hypermutation and class switching to IgG and IgA, confirming that this is antigen-driven autoimmunity.
- Epithelial cell dysfunction and cytokine-mediated inflammation: The primary target tissue—salivary and lacrimal epithelium—is not merely a passive victim but an active participant in disease pathogenesis. Sjögren's epithelial cells upregulate expression of adhesion molecules (ICAM-1, LFA-1) and chemokines (MCP-1, RANTES, IL-8) that recruit lymphocytes into the glands. Additionally, epithelial cells express HLA-DR and HLA-DQ molecules, acting as antigen-presenting cells (APCs) that activate infiltrating T cells in situ. The cytokine milieu within affected glands is heavily skewed toward Th1 and Th17 responses, with elevated production of IFN-γ, TNF-α, IL-17, and IL-6. These pro-inflammatory cytokines directly impair secretory function by disrupting tight junction proteins (claudins, occludin), reducing aquaporin-5 water channel expression (critical for saliva and tear production), and triggering apoptosis of acinar and ductal epithelial cells through death receptor signaling (Fas/FasL pathway).
- Lymphocytic infiltration and ectopic germinal center formation: Histologically, affected salivary and lacrimal glands show replacement of normal acinar tissue by dense lymphocytic infiltrates, predominantly CD4+ T cells and B cells, with a CD4:CD8 ratio often exceeding 1.5:1. Advanced stages demonstrate ectopic germinal center-like structures within gland tissue, indicating ongoing clonal B cell selection and maturation in the target organ itself. This local B cell activation is supported by follicular helper T cells (TFH) that produce IL-21, driving further B cell differentiation and antibody production. Over time, progressive destruction of acinar units leads to functional deterioration of secretion, as remaining epithelial cells cannot sustain adequate fluid output due to mitochondrial dysfunction and impaired ductal reabsorption mechanisms.
- Autoantigen molecular mimicry and epitope spreading: Initial loss of tolerance is thought to be triggered by viral infection, most notably with Epstein-Barr virus (EBV) or cytomegalovirus (CMV), which display epitopes homologous to Ro52 and La/SS-B. Viral replication within epithelial cells triggers innate immune activation through pattern recognition receptors (TLRs, particularly TLR3 and TLR7 sensing viral RNA), leading to IFN-α and IFN-β production. The interferon signature—elevated type I interferon activity measurable by serum interferon-α levels and interferon-inducible gene expression (IP-10, MxA, OAS)—perpetuates both innate and adaptive immunity. Once anti-Ro52 responses are initiated, epitope spreading occurs such that antibodies and T cells expand to recognize the La/SS-B antigen, which physically associates with Ro52 in ribonucleoprotein complexes. This explains why anti-La antibodies are almost always found in conjunction with anti-Ro52 antibodies.
- Contribution of genetic variants in lymphoid signaling pathways: Polymorphisms in genes encoding molecules critical for B cell survival and activation amplify the autoimmune response. For example, variants in BLK (B lymphocyte kinase) reduce the threshold for B cell receptor signaling, facilitating activation of self-reactive clones. Similarly, STAT4 polymorphisms enhance IL-12 and IL-23 signaling, promoting Th1 and Th17 differentiation. These genetic variants, combined with environmental triggers and loss of tolerance, create a permissive landscape for sustained B cell-driven autoimmunity characteristic of Sjögren's syndrome.
- Genetic predisposition (HLA and non-HLA loci): The strongest risk factor for Sjögren's syndrome is inheritance of specific HLA alleles, particularly HLA-B8 and HLA-DR3 in Caucasian populations, and HLA-DQ2 associations in multiple ethnic groups. Non-HLA genetic risk factors include polymorphisms in IRF5 (interferon regulatory factor 5), STAT4 (signal transducer and activator of transcription 4), and BLK genes, each contributing modest increases in disease susceptibility. Genome-wide association studies have identified additional loci involved in immune checkpoint control and B cell activation, accounting for approximately 50% of genetic heritability. Family members of patients with Sjögren's syndrome have increased serologic markers of autoimmunity, though clinical disease penetrance remains incomplete, underscoring the necessity of environmental cofactors.
- Female sex and hormonal factors: The striking 9:1 female predominance in Sjögren's syndrome points to critical roles for estrogen and other sex hormones in disease pathogenesis. Estrogen enhances B cell survival through increased BAFF production, promotes Th17 differentiation through IL-23 signaling, and impairs Treg development. Androgens, conversely, are protective through their immunosuppressive effects on B cells and proinflammatory T cells. Onset frequently occurs perimenopausally or in postmenopausal women, consistent with the hypothesis that declining androgen levels (even in women, androgens are produced in the adrenal glands) remove a brake on autoimmunity. Some studies suggest that estrogen replacement therapy may exacerbate disease activity, though the evidence remains mixed.
- Environmental triggers: Viral infections, particularly Epstein-Barr virus (EBV), have been implicated through multiple lines of evidence: (1) elevated anti-EBV antibodies and increased EBV reactivation in Sjögren's patients; (2) EBV-infected B cells present in affected salivary glands; and (3) molecular mimicry between EBV antigens and Ro52/La epitopes. Hepatitis C virus infection is associated with increased prevalence of sicca symptoms and anti-Ro/La seropositivity, particularly in Mediterranean populations. Other proposed triggers include exposure to silica dust (occupational exposure), though epidemiologic evidence is modest, and possibly therapeutic interferons (reports of Sjögren's-like disease developing during IFN-α treatment for hepatitis C).
- Secondary Sjögren's syndrome: Development of Sjögren's features in the setting of another autoimmune disease defines secondary Sjögren's syndrome. The most common associations are with rheumatoid arthritis (10-30% of RA patients have features of Sjögren's), systemic lupus erythematosus (15-20% prevalence), and systemic sclerosis (5-15%). Patients with sSS typically have earlier-onset disease, more severe systemic manifestations, and a worse prognosis compared to primary Sjögren's. The presence of HLA-DR4 (in RA-associated Sjögren's) or anti-dsDNA antibodies (in SLE-associated Sjögren's) indicates different underlying immunopathology tailored to the primary autoimmune condition.
- Prior graft-versus-host disease (GVHD): Chronic GVHD following allogeneic hematopoietic stem cell transplantation can produce Sjögren's-like sicca syndrome through donor T cell attack on host salivary and lacrimal tissue. This condition is termed chronic GVHD with sicca features and may be difficult to distinguish clinically and serologically from primary Sjögren's syndrome. However, sicca attributable to GVHD typically lacks anti-Ro/La antibodies and demonstrates different HLA associations, helping to differentiate it from true Sjögren's syndrome.
- Xerophthalmia (dry eyes): The hallmark ocular manifestation is dryness caused by impaired lacrimal gland secretion, leading to loss of the aqueous tear layer. Patients report irritation, grittiness, foreign body sensation, and paradoxically excessive tearing (reflex tearing in response to corneal irritation). Symptoms typically worsen throughout the day, in dry environments, and with screen time (which reduces blink rate). Physical examination may reveal a positive Schirmer test (wetting <5 mm of filter paper in 5 minutes without topical anesthetic), though this test lacks specificity. More specific is ocular staining, wherein fluorescein or rose bengal dye highlights areas of corneal and conjunctival epithelial damage (positive vital dye staining); this reflects actual tissue injury from chronic desiccation and is more closely associated with systemic disease activity than tear flow alone.
- Xerostomia (dry mouth): Reduced salivary gland function results in oral dryness, difficulty swallowing dry foods, impaired taste sensation (due to loss of saliva's solvent function), and increased dental caries and oral infections. Patients may describe difficulty eating unless they take fluids with meals, and they often report needing to sip water frequently throughout the day. Severe xerostomia can impair speech clarity and reduce oral comfort significantly, affecting quality of life. The dry mouth results directly from destruction of salivary acini and ducts, compounded by loss of neural stimulation due to parasympathetic denervation from immune attack on salivary gland innervation.
- Salivary gland enlargement (parotitis): Approximately 50% of Sjögren's patients develop painless, bilateral parotid gland swelling, reflecting both lymphocytic infiltration and acinar destruction with residual reactive hyperplasia. Enlargement typically affects the main parotid gland bilaterally but may involve submandibular glands. When pronounced, parotitis can be mistaken clinically for mumps (though serologies differ) or early lymphoma. Notably, parotid swelling may wax and wane or may be a presenting feature that precedes sicca symptoms by months to years, making it an important early clinical clue to Sjögren's syndrome diagnosis.
- Systemic features - arthralgia and non-erosive arthritis: Approximately 70% of Sjögren's patients experience arthralgias, and 30-40% develop frank arthritis, typically affecting small joints of the hands (PIP and MCP joints), wrists, and knees. The arthritis is non-erosive, meaning that plain radiographs show no joint space narrowing or erosions, distinguishing Sjögren's arthritis from rheumatoid arthritis (though RA and Sjögren's can coexist in secondary Sjögren's). Joint inflammation reflects infiltration of synovium by autoreactive T cells and local production of pro-inflammatory cytokines (TNF-α, IL-6). Notably, in some patients, arthritis may precede sicca symptoms or may dominate the clinical picture.
- Systemic features - pulmonary involvement: Lung disease occurs in 20-80% of Sjögren's patients depending on detection method used. Common manifestations include interstitial lung disease (ILD), typically a nonprogressive, lymphocytic interstitial pneumonia (LIP) pattern on imaging. Less common are bronchiectasis, bronchitis sicca (dry cough), and bronchial hyperresponsiveness (asthma-like symptoms). Pulmonary disease may be subclinical, detectable only on high-resolution CT or pulmonary function testing (typically showing a restrictive pattern with reduced DLCO), or may cause significant dyspnea on exertion. The mechanism involves lymphocytic infiltration of the lung interstitium with production of IL-17 and TNF-α driving inflammation and epithelial damage.
- Systemic features - renal involvement: Renal manifestations occur in 25-30% of patients and include interstitial nephritis (the most common form) and membranoproliferative glomerulonephritis (MPGN). Interstitial nephritis presents with mild to moderate proteinuria, hematuria, and gradual decline in GFR, often asymptomatic until advanced. The mechanism is CD4+ T cell-mediated interstitial inflammation with granuloma formation in some cases. More rarely, immune complex-mediated MPGN can develop, especially in patients with high-titer anti-Ro/La antibodies, leading to more aggressive kidney disease. Distal renal tubular acidosis (dRTA or Type 4) is a distinctive finding in Sjögren's, occurring in 10-15% of patients, wherein reduced ability to acidify urine leads to non-anion-gap metabolic acidosis, hypokalemia, and kidney stone formation.
- Systemic features - hepatic involvement: Liver disease is present in up to 40% of patients histologically but is usually mild and asymptomatic. Primary biliary cirrhosis (PBC) and primary sclerosing cholangitis (PSC) occur with increased frequency in Sjögren's syndrome compared to the general population. Patients may show elevated alkaline phosphatase and anti-mitochondrial antibodies (AMA) without clinical PBC. True symptomatic hepatic disease with cirrhosis is less common but carries significant prognostic implications.
- Systemic features - neurologic involvement: Neurologic manifestations develop in 20-25% of patients and include both central nervous system (CNS) and peripheral nervous system (PNS) involvement. Peripheral neuropathy, predominantly sensory or sensorimotor, affects 10-20% and may be axonal or demyelinating; trigeminal neuropathy (causing facial numbness or pain) is a relatively specific neurologic manifestation of Sjögren's. CNS involvement is less common but can present as cognitive dysfunction, myelitis, or optic neuritis, resembling multiple sclerosis clinically. The mechanism involves lymphocytic infiltration of peripheral nerves and/or immune-mediated vasculitis of vasa nervorum.
- Systemic features - cutaneous involvement: Skin manifestations occur in 30-50% of patients and include palpable purpura (small-vessel vasculitis affecting skin), photosensitive dermatitis (exacerbated by sun exposure), and annular erythematous lesions resembling those of lupus. Some patients develop cryoglobulinemia with skin involvement as part of systemic manifestations. The presence of cutaneous vasculitis is associated with more severe systemic disease and increased risk of lymph
Step 1 — confirm objective sicca (not just symptoms)
- Schirmer test: ≤5 mm of wetting in 5 minutes (without anesthetic) indicates aqueous-deficient dry eye. Cheap and quick, but poorly specific.
- Ocular staining score: fluorescein/lissamine green staining of cornea and conjunctiva demonstrates actual epithelial damage; a high score (van Bijsterveld ≥4 or OSS ≥5) is one of the weighted classification items.
- Unstimulated whole salivary flow: ≤0.1 mL/min supports true hyposalivation rather than subjective dryness.
Step 2 — serology
- Anti-Ro/SSA: the single most useful antibody and the one that carries the heaviest weight in classification. Anti-La/SSB in isolation is not considered diagnostic in the current criteria because it lacks specificity.
- Supportive but nonspecific: speckled ANA, rheumatoid factor, polyclonal hypergammaglobulinemia, and low C3/C4 (the latter a poor-prognosis marker).
Step 3 — histology (reference standard when serology is negative)
- Labial minor salivary gland biopsy: focal lymphocytic sialadenitis with a focus score ≥1 (one focus of ≥50 lymphocytes per 4 mm² of glandular tissue). This is required in seronegative patients and is the most specific single finding.
Criteria: The 2016 ACR/EULAR classification criteria assign weighted points across these five items (focus score ≥1 and anti-Ro/SSA are weighted most heavily; Schirmer, ocular staining, and salivary flow less so), with a total score at or above the defined threshold classifying the patient. These are classification criteria for research cohorts, applied clinically as a diagnostic framework. Mandatory exclusions must be ruled out first: hepatitis C, HIV, sarcoidosis, amyloidosis, IgG4-related disease, prior head/neck irradiation, chronic GVHD, and active anticholinergic drug use.
- Adjuncts: salivary gland ultrasound (heterogeneous, hypoechoic glands) is increasingly used but is not a criteria item. Cross-sectional imaging is reserved for suspected lymphoma.
Therapy is symptom-directed for glandular disease and immunosuppressive only for systemic disease; no agent reverses established acinar destruction. US practice follows the Sjögren's Foundation clinical practice guidelines, with EULAR recommendations widely referenced for systemic manifestations.
Ocular dryness (stepwise)
- Preservative-free artificial tears and gels: first-line, plus environmental measures (humidification, reduced screen time, avoidance of ceiling fans).
- Topical immunomodulators: cyclosporine ophthalmic emulsion or lifitegrast (LFA-1 antagonist) for inflammatory dry eye unresponsive to lubricants.
- Short-course topical corticosteroids and punctal occlusion under ophthalmology supervision; autologous serum tears for refractory keratitis.
Oral dryness
- Nonpharmacologic: sugar-free xylitol lozenges/gum, saliva substitutes, meticulous dental hygiene, and topical fluoride with frequent dental follow-up — caries prevention is a core recommendation, not an afterthought.
- Muscarinic agonists: pilocarpine or cevimeline stimulate residual M3 receptors on acini; expect sweating, flushing, and GI cramping. Avoid in uncontrolled asthma/COPD, narrow-angle glaucoma, and significant bradycardia.
Systemic disease
- Hydroxychloroquine: commonly used for arthralgia, fatigue, and cutaneous disease, though randomized evidence for symptom benefit has been disappointing.
- Glucocorticoids for active organ-threatening inflammation, with a steroid-sparing agent (methotrexate, azathioprine, or mycophenolate) for arthritis, ILD, or interstitial nephritis.
- Rituximab (anti-CD20) is reserved by EULAR for severe refractory extraglandular disease — cryoglobulinemic vasculitis, severe neuropathy, progressive ILD — reflecting the B-cell-driven pathogenesis. Cyclophosphamide for life-threatening vasculitis.
Avoid
- Anticholinergic burden: first-generation antihistamines, tricyclics, oxybutynin, and some antipsychotics worsen sicca and should be substituted.
- Systemic steroids are not appropriate maintenance therapy for isolated sicca.
- Alkali replacement (potassium citrate) is required for distal RTA; potassium-wasting must be corrected before it becomes symptomatic.
Malignant
- Non-Hodgkin lymphoma, classically MALT lymphoma of the parotid: chronic ectopic germinal-center activity drives clonal B-cell selection. Sjögren's carries the highest lymphoma risk of any autoimmune rheumatic disease. Red flags: persistent or unilateral parotid enlargement, new lymphadenopathy or splenomegaly, palpable purpura, cryoglobulinemia, low C4, a monoclonal band, and paradoxical fall in previously high rheumatoid factor or gammaglobulins. Any of these mandates imaging and tissue biopsy.
Emergencies
- Cryoglobulinemic vasculitis: immune-complex small-vessel injury causing purpura, mononeuritis multiplex, and glomerulonephritis; hyperviscosity is a medical emergency.
- Severe hypokalemia from distal RTA: impaired distal H⁺ secretion drives non-anion-gap acidosis with K⁺ wasting; can present as hypokalemic paralysis or arrhythmia, plus nephrocalcinosis/stones.
- Corneal ulceration or perforation from untreated keratoconjunctivitis sicca — sight-threatening; requires same-day ophthalmology.
- Transverse myelitis or optic neuritis (overlaps with NMOSD; check aquaporin-4 IgG).
Obstetric
- Neonatal lupus and congenital complete heart block: maternal anti-Ro/SSA (and anti-La) IgG crosses the placenta and injures the fetal AV node — irreversible once complete. The ACR reproductive health guideline supports hydroxychloroquine in anti-Ro-positive pregnancies and serial fetal echocardiography in the mid-second trimester.
Other disease complications: rampant cervical dental caries, oral candidiasis with angular cheilitis, esophageal dysphagia, progressive ILD, and interstitial nephritis with CKD.
Treatment complications
- Hydroxychloroquine retinopathy: dose- and duration-dependent; requires baseline and annual screening per ophthalmology recommendations.
- Muscarinic agonists: sweating, flushing, bradycardia, bronchospasm.
- Rituximab: infusion reactions, hypogammaglobulinemia, hepatitis B reactivation (screen before use), rarely PML.
- Antimetabolites/cyclophosphamide: cytopenias, infection, and with cyclophosphamide hemorrhagic cystitis and infertility.
- The classic stem: a middle-aged woman with gritty/sandy eyes, needing water to swallow crackers, bilateral parotid fullness, and rampant dental caries. Dry eyes + dry mouth + non-erosive arthritis is the sicca triad framing examiners use.
- The antibody to know: anti-Ro/SSA is the workhorse. Anti-La/SSB almost never occurs alone and is not sufficient by itself in the 2016 ACR/EULAR criteria. Expect a speckled ANA, positive rheumatoid factor, and polyclonal hypergammaglobulinemia — RF positivity here is a classic distractor pointing wrongly toward RA.
- Best next step when serology is equivocal: labial minor salivary gland biopsy showing focal lymphocytic sialadenitis with focus score ≥1 per 4 mm².
- Best next step for persistent or asymmetric parotid enlargement: imaging plus biopsy to exclude MALT lymphoma — not another course of antibiotics. A falling RF titer or new low C4 with purpura in a known Sjögren's patient is the lymphoma cue.
- The one association examiners love: maternal anti-Ro/SSA → neonatal lupus and congenital complete heart block. A bradycardic fetus at ~18–24 weeks in an asymptomatic mother should prompt maternal anti-Ro/La testing.
- Renal buzzword: distal (type 1) RTA — non-anion-gap metabolic acidosis, hypokalemia, urine pH inappropriately >5.5, nephrolithiasis. Do not label this type 4; type 4 is hyperkalemic.
- Drug pearl: pilocarpine/cevimeline are muscarinic agonists — contraindicated in uncontrolled asthma and narrow-angle glaucoma. Conversely, anticholinergics (diphenhydramine, TCAs, oxybutynin) are the drugs to stop.
- Mimics to exclude before diagnosing: hepatitis C, HIV, sarcoidosis, amyloidosis, IgG4-related disease, prior head/neck radiation, and chronic GVHD. Bilateral parotid swelling with hypercalcemia and hilar adenopathy is sarcoid, not Sjögren's.
